How to Enroll in WIC

Applicant/participant must be present and MUST BRING:

• Proof of ID for all participants 
  For example: Driver’s license, official photo ID, Medicaid card, birth certificate, passport, etc. Also acceptable for newborns are Mothers verification of facts, affidavit of parentage, birth certificates, crib card, hospital bracelets, discharge paperwork for baby.

• Proof of North Carolina residence (must include current physical address)
  For example: Utility bill less than 60 days old (electric, gas, phone, water, cable), current rental/mortgage agreement.

• Proof of income
  Including most recent Military Leave and Earnings Statement (LES), pay stubs (4 weeks), unemployment, Medicaid Card, letter of Food Stamp eligibility, etc.
  
  * Participants with documentation of eligibility for Medicaid, Food and Nutrition Services (FNS) Program (Food Stamps) and/or the Temporary Assistance to Needy Families (TANF) Program (Work First) are income eligible for WIC. 
  

• For infants proof of identity  
  Documents provided at the hospital: Mother's Verification of Facts Document, Crib Card, Hospital Band or Shot Record.  

• Doctor's referral form with infant’s measurements 
  Information must be less than 60 days old (infant’s height, weight and Hemoglobin, if needed).  If a referral form from the provider is not available, the WIC staff will collect the infant’s measurements in the office.

• Also Requested
  Shot record(s) and Lead Screening documentation for the infant.
